Proof of Eligibility Documents:

You can use the “Document” section of the Device Request form to send us your proof of eligibility immediately if you already have a qualifying document available.

Documents uploaded MUST be in one of the following file formats:

.pdf, .png, .jpg

Your document should include the following information:

✳️Same name and age as applicant

✳️A date or date range showing CURRENT & ACTIVE coverage/support

✳️Name and contact information of supporting organization

***Identification cards do NOT count as documents in this context***
